HOW TO BECOME A MEMBER

 

We are pleased that you are interested in joining the Kentucky Society of Oral & Maxillofacial Surgeons (KSOMS).  The KSOMS is a component society of the American Association of Oral and Maxillofacial Surgeons (AAOMS). Membership in a state component society is necessary in order to become a member in AAOMS. In order to become a member of the KSOMS, a candidate for membership must have graduated from dental school and have completed Oral and Maxillofacial Surgery training. Board certification by the American Board of Oral and Maxillofacial Surgery is not required but is encouraged.  

The following is a list of prerequisites, which need to be forwarded to Dr Richard Pape, KSOMS President. Please mail your completed application and supporting materials to KSOMS, PO Box 753, Crestwood, Kentucky 40014.

 

  1. A completed KSOMS membership application. To download an application click here.
  2. Two letters of recommendation from oral and maxillofacial surgeons who know you personally. These should be on their letterhead.
  3. A check for $75.00 to cover the costs to process your application and your on-site office anesthesia evaluation.
  4. A letter on file with Dr Richard Pape requesting an on-site office anesthesia evaluation.  (This is separate from the office evaluation performed by Kentucky Board of Dentistry and is required for membership in the KSOMS.)  Send this to KSOMS, PO Box 753, Crestwood, Kentucky 40014-0753 and keep a copy of this for your records.
